她從小在台灣貢寮海邊長大，

原本單純的生活樂趣，卻在過去十多年，

Ms. Tsai, 42, older sister of Taiwanese actor Emerson Tsai (Chang-Hsien Tsai),
grew up in the coastal town of Gongliao, Taiwan,
where fresh seafood was a way of life.
Her favorites?
Hard-shell seafood—crabs, lobsters, anything you have to crack open.
But over the past decade,
that simple joy has been chipped away by relentless dental problems.

被評估需拔除的牙 在蒔美找到希望 A Tooth Marked for Extraction — Until Smile Dental Stepped In
直到某次門牙長出膿包，住處附近的家庭牙醫建議她至大醫院做顯微根管。她陸續看了幾家診所與醫院，卻得到幾乎一致的結論：「牙根太差，建議拔除。」
When an abscess appeared near her front tooth, her local dentist referred her to a hospital for Microscopic Endodontic Surgery. Several specialists reviewed her case and reached the same conclusion: the roots were too damaged to save. Extraction seemed unavoidable.
正當她感到心灰意冷時，在弟弟的推薦下，來到蒔美牙醫。沒想到，江政倫院長詳細評估後，卻給出完全不同的診斷：「雖然技術較為複雜，但這兩顆牙我們可以幫你保留下來。」
Feeling discouraged, she followed her brother’s advice and visited Smile Dental Clinic. To her surprise, Dr. Cheng-Lun Chiang offered a different assessment. “It won’t be easy,” he said, “but we can save these teeth.”

Preserving Front Teeth for Lasting Stability and Beauty
江醫師指出，受損嚴重的兩顆門牙若直接拔除，並不一定能獲得最理想的修復效果。由於前牙區的齒槽骨天生本就較薄，加上患者骨質條件偏弱，若同時拔除兩顆牙齒，將可能影響齒槽根尖的癒合，並增加牙齦萎縮的風險，導致後續重建的美觀與穩定度打折扣。因此，保留這兩顆牙齒成為更合適的選擇。
Two of Ms. Tsai’s front teeth were fractured and seemed destined for extraction. But Dr. Chiang chose a different path. Removing them would have risked gum loss and a weaker foundation for future restorations.
這兩顆牙之所以裂開，主要源自於患者嚴重的深咬。深咬對牙齒的長期傷害極大，因此治療的首要任務便是先改善深咬，減輕牙齒承受的過度壓力。
Years of deep bite had caused the damage, so the treatment began by correcting her bite and easing the pressure on her teeth.
此外，為確保保留的門牙能長久使用，牙根需在骨頭內至少保留 8mm 的長度，並且在牙齦上方還需有 2mm 的齒質 才能穩固支撐重建的假牙。透過平面 X 光與 3D 電腦斷層的多角度測量分析，我們確認蔡小姐的條件符合要求，因此能順利執行此療程。
至於同樣受損的側門牙，雖然狀況更為嚴重，但透過咬合的重新設計與調整，仍能有效降低其受力，延長保存的可能。
綜合評估軟組織癒合的可行性、牙根健康度、咬合受力分配以及贗復體修復設計等多重因素，我們最終決定以 原齒保留 取代 植牙治療，為患者保留更多自然牙結構，也為後續的功能與美學表現奠定基礎。
Imaging showed just enough healthy root and tooth structure to make preservation possible. Even a badly damaged incisor could be stabilized by carefully redesigning her bite.
Rather than rushing to implants, the plan focused on saving her natural teeth—protecting both their function and aesthetics for the long term.
在錯誤的咬合上重建，難怪越治越差 The Hidden Culprit: Rebuilding on a Faulty Bite
事實上，成功保住門牙，只是重建之路的一小步。要真正解決蔡小姐長年牙齒反覆出問題的根源，必須回到整體咬合系統重新檢視。
Saving her front teeth was just the first step. To solve her long-standing dental problems, Dr. Chiang needed to look deeper at her bite.
經由江院長的全面檢查，發現問題並不只是某幾顆牙的局部狀況，而是整體咬合早已嚴重失衡。蔡小姐長期因壓力與緊張造成夜間磨牙，加上喜愛啃食硬殼海鮮，使牙齒長年過度磨耗，情況愈演愈烈。咬合平面一旦紊亂，牙齒之間彼此干擾，不僅真牙受損，連假牙也會失去密合度，反覆脫落、發炎，怎麼修都無法長久。
Dr. Chiang's comprehensive evaluation revealed that the problem wasn’t with individual teeth, but with her entire bite system.
Years of night grinding—made worse by stress and her love of hard-shell seafood—had worn down her teeth severely.
Once her bite became misaligned, her teeth interfered with each other. Both natural teeth and prosthetics loosened, became inflamed, and repeatedly failed.

“If we keep repairing one tooth at a time, the cycle will just continue,” Dr. Chiang explained. “To break it, we have to address the bite itself.”

Because years of treatment and shifting bite patterns had left the bone in her lower right jaw too weak to withstand normal chewing, Dr. Chiang made the call to remove the failing tooth. To restore her bite, he turned to the Swiss-made Straumann® TLX implant system.
一般植牙多採用 10mm 以上長度的植體以增強穩定性，但在蔡小姐骨質與骨量有限的情況下，江醫師選擇了 8mm 與 6mm 的 Straumann® TLX 植體。這些短植體之所以能在嚴苛條件下仍提供穩固支持，關鍵在於其專利技術：
While most implants rely on posts longer than 10 millimeters for stability, her limited bone structure meant that only 8mm and 6mm fixtures could be used. What made this possible were Straumann’s proprietary designs—short implants engineered to remain secure even under difficult conditions.

Roxolid®: A Stronger Alloy, Less Need for Bone Grafts
Made from a titanium-zirconium blend that combines biocompatibility with exceptional strength, Roxolid® is considered one of the most durable implant materials available. Studies show it integrates with bone more reliably than conventional titanium. Paired with Straumann’s deep-thread and extended-groove design, the alloy helps lock implants securely into place while preserving more of the patient’s natural bone. In practice, it means that even shorter, narrower implants can provide lasting stability—reducing the need for invasive bone grafts and allowing for a less traumatic procedure overall.
SLActive® 親水表面處理：加速癒合、降低發炎風險
能在植入後的黃金癒合期迅速吸引血液中的蛋白質與骨細胞，加速骨再生。相較於傳統表面約需四週才能進入穩定狀態，SLActive® 植體僅需兩週即可達到，縮短了最不穩定的恢復期，並降低早期失敗風險。更進一步，它能調控免疫反應，誘導巨噬細胞產生抗炎微環境，減少致炎因子的釋放，降低植體周圍炎的發生率。文獻指出，Straumann® 植體的十年周圍炎發生率甚至不到 2%，僅為其他大廠的三分之一。

By drawing in proteins and bone cells during the crucial early weeks after surgery, SLActive® creates a surface that helps bone regenerate more quickly. Where traditional implants may take a month to stabilize, SLActive® often reaches that stage in just two weeks, shortening the most fragile part of recovery and reducing the risk of early failure. The surface also shapes the body’s immune response, fostering an anti-inflammatory environment that lowers the chance of peri-implantitis. Long-term studies show the difference is striking: after ten years, fewer than 2 percent of Straumann® implants develop this complication—about a third of the rate reported by other leading brands.

Dr. Chiang chose Straumann’s TLX Tissue Level implant, designed with a machined collar that lowers the risk of peri-implantitis while withstanding chewing forces. The trade-off, however, is precision: the implant must be placed so the gum tissue seals perfectly around its neck. To achieve that accuracy, the team used 3D scans, digital planning and X-Guide navigation, ensuring each implant was set in the ideal position for both strength and aesthetics.
